INFORMATION FOR GOVERNMENT MONITORING
PURPOSES
The following information is requested
by the Federal Government for certain types of loans related
to a dwelling, in order to monitor Star One's compliance
with equal credit opportunity, fair housing and home mortgage
disclosure laws. You are not required to furnish this
information, but are encouraged to do so. The law
provides that Star One may neither discriminate on the basis
of this information, nor on whether you choose to furnish
it. However, if you choose not to furnish it, under
Federal regulations Star One is required to note race and
sex on the basis of visual observation or surname.
If you do not wish to furnish the information, please check

Thank you for applying for a Star One Credit Union home
loan. Please return the following items to Star One as soon
as possible.
1. Payment of $450 to Star One. Please select
one of the payment options below:
($375.00 is a non refundable fee for
the appraisal; this amount may vary, depending upon your
property and the type of loan. The other $75.00 is an application
fee, which is refundable at the time your loan closes. Payment
of $75.00 must be received to process your application.
If your loan does not close for any reason, the fee will
be retained by Star One.)
Please note that there will be additional
charges for this loan: Title insurance, escrow, prepaid
interest and other closing costs. An estimate of your closing
costs will be mailed to you in the next three business days.
2. We can recommend
a title and/or escrow company and escrow officer, but if
you would prefer, you may select your own.

Homeowner's insurance coverage must be equal to the replacement
value of the improvements or the loan balance, whichever
is less. We cannot require you to maintain hazard
insurance in excess of the value of the improvements.
Star One will provide an estimate of
costs within 3 business days of the receipt of this application.
