Electronic Signatures Disclosure

The Electronic Signatures in Global and National Commerce Act (E-Sign Act) allows Star One Credit Union to provide you with documents or communications in electronic form instead of written form and allows you to use your electronic signature in lieu of signing a paper document upon your consent and after we have provided you with the following information.

Please read the following information thoroughly. By proceeding with and submitting this account application, you are agreeing that you have read the following Electronic Disclosure and Consent information and consent to transact business using electronic communications, to receive notices and disclosures electronically, and to utilize electronic signatures in lieu of using paper documents. This electronic disclosure service is provided on behalf of Star One Credit Union, who is sending you electronic documents, notices, disclosures, or requesting electronic signatures. You are not required to receive notices and disclosures or sign documents electronically. If you prefer not to do so, you may request to receive paper copies and withdraw your consent at any time as described below.

Scope of Consent

You agree to receive electronic notices, disclosures, and electronic signature documents with all related and identified documents and disclosures provided over the course of your relationship with Star One Credit Union. You may, at any point withdraw your consent by following the procedures described below under Withdrawal of Consent.

Paper Copies

You are not required to receive notices, disclosures, or sign documents electronically and may request paper copies of documents or disclosures if you prefer. You also have the ability to download and print any opened or signed documents sent to you through the electronic signature service. You may access your notices, disclosures, or signed documents as Internet web PDF or HTML documents (images). If you wish to receive paper copies in lieu of electronic documents, you may request paper copies from Star One Credit Union by following the procedures outlined below. There are no fees or account restrictions for requesting to receive documents, notices, or disclosures in paper format, choosing to withdraw your consent for eStatements and eNotices, or changing your email address. Reproduction of documents such as past account statements may incur a processing fee.

Hardware and Software Requirements

In order to receive and save electronic notices, disclosures, or sign documents electronically, you must have a computer, mobile device, or other electronic device with internet access. You must have a functional, certified internet browser and Adobe Acrobat Reader software to access the notices, disclosures, or signed documents and a printer or ability to download the statements for your records. Use of this Service requires a standard-compliant web-browser, which supports the HTTPS protocol, HTML, and cookies. Viewing PDF documents require additional software such as Adobe Reader or similar. You must also have an active e-mail address. You will be notified of any changes to the hardware and/or software requirements that may create a material risk that you will not be able to receive, view, print, or save a document or communication.

Withdrawal of Consent

You may withdraw your consent to receive electronic documents, notices, or disclosures at any time. In order to withdraw consent, you must notify Star One Credit Union that you wish to withdraw consent and to provide your future documents, notices, and disclosures in paper format. After withdrawing consent if at any point in the future you proceed forward and utilize the electronic signature system you are once again consenting to receive notices, disclosures, or documents electronically. You may withdraw consent once more by following the procedures described below.

Requesting paper copies, withdrawing consent, and updating contact information

To request paper copies of documents, withdraw consent to conduct business electronically and receive documents, notices, or disclosures electronically, or sign documents electronically, please contact Star One Credit Union by telephone at 1-408-543-5202 or 1-866-543-5202, or by postal mail to Star One CU P.O. Box 3643 Sunnyvale, CA 94088-3643. Online Banking users may also submit a request through Secure Support by clicking on the Support button. You are responsible for informing Star One Credit Union of any change to your e-mail address, or mobile phone number.

• Requesting Paper Copies—Provide your name, email, telephone number, postal address, and document title.
• Withdraw Consent—Provide your name, email, date of request, telephone number, and postal address.
• Update Contact Information—Provide your name, email, telephone number, and postal address.
