About Sole Proprietorship Accounts
A Sole Proprietorship Account can only be established within a new membership that is separate from all other Star One memberships that you might have.
The name of your sole proprietorship must contain your surname.
For example, "Cox Auto Services" would be an acceptable sole proprietorship name at Star One, whereas "Family Auto Services" would not.
Existing members can open a new membership or update an existing membership using this form.
This form requires a notarized signature.

Instructions
For existing Star One members only:
- Read and accept the two disclosures provided
- Fully complete the form
- Print the form
- Sign the form in front of a notary
- Photocopy your valid U.S. driver's license or State-issued ID card
- Mail the photocopy, supporting documents, and printed form to Star One Credit Union, P.O. Box 3643, Sunnyvale CA 94088-3643
To avoid processing delays, and to collect your most current information, the Member Application and Account Card must be completed in its entirety.
